Osteospinal™

Structured reasoning for safer, better outcomes.

A clinical reasoning support tool for spinal care practitioners — osteopaths, physiotherapists, and chiropractors. Ask better questions. See more. Miss less.

See more. Miss less.

👤 For registered practitioners only — Osteospinal is designed for use by registered spinal care practitioners. The clinical output uses professional terminology and is not intended for independent patient use. If you are a patient, your practitioner will complete the assessment and share a plain-language summary with you.

Osteospinal supports clinical decision-making. It does not provide diagnoses and does not replace professional judgement. All clinical decisions remain with the practitioner.

Why Osteospinal works, for patients and clinicians

Back pain is the most common reason people see GPs, osteopaths, physiotherapists, and chiropractors. The vast majority of cases are straightforward and entirely within scope. But a small, important minority are not — and the profession can fail these patients when systematic reasoning gives way to pattern recognition alone.

Cognitive bias affects all clinicians. Anchoring, search satisfaction, and availability bias can all lead to premature conclusions — not through lack of skill, but through the inherent difficulty of the task. The gap is rarely knowledge. It is structure.

Osteospinal is designed for situations such as:

  • Red flags requiring urgent medical referral
  • Musculoskeletal versus inflammatory pain patterns
  • Neurological features suggesting nerve root compression or cauda equina involvement
  • Visceral and systemic causes that can mimic spinal pain
  • Sciatica imposters — leg pain without a spinal nerve root cause
  • Presentations that are not progressing as expected

Two reports. One assessment.

Every completed assessment generates two separate reports — one for you, one for your patient.

Clinician Report

Confidence-weighted differentials grouped by clinical gravity, treatment considerations, contraindications, clinical reasoning prompts with named examination tests, and a structured progress review for follow-up.

📋

Patient Summary

A personalised plain-language report that connects the patient's answers to the clinical findings. What it is, why the answers pointed to it, what it means for care, and what to expect. Written to build understanding and trust.

⛔ UrgentMedical attention required — do not treat until assessed
⚠ ImportantGP or specialist review — outside scope of spinal care alone
🥴 Musculoskeletal (spinal structures)Primary clinical considerations — within scope of practice
💬 PeripheralContributing factors — non-threatening, explains symptoms

Not all findings carry equal weight. Osteospinal distinguishes between what needs urgent attention, what needs careful monitoring, and what is within scope — every time, for every patient.

A powerful differentiator

Most patients assume all clinics are the same. Osteospinal gives member clinics something others do not have — a structured, safety-first assessment system that is visible to patients and demonstrates a commitment to thorough, thoughtful care.

🛡

Reduce clinical risk

Systematic checks prevent “treat first, realise later” scenarios. The tool prompts the questions that change clinical behaviour.

🔗

Improve patient safety

Clear escalation cues help you refer when needed and treat when appropriate. Gravity-weighted output means urgent features are never buried.

📈

Better outcomes

Earlier clinical clarity leads to better-directed treatment and faster recovery trajectories.

Save consultation time

You start every consultation with structured insight, not guesswork. The report generates in seconds.

For patients

If your practitioner uses Osteospinal, they are using a structured clinical reasoning tool to make sure your back pain is assessed thoroughly and safely.

  • Safer clinical decisions — your assessment is structured, not assumed
  • Clearer explanations — a plain-language summary explains what was found and why
  • More appropriate care pathways — the right treatment, not just the most familiar one
  • Improved confidence — you understand your condition, not just your symptoms

The assessment takes around five minutes. Your answers are used to generate a personalised summary — written in plain language — that explains what patterns your practitioner identified, what they mean for your care, and what to expect from your recovery.

All clinical decisions remain with your practitioner. If you have questions about anything in your summary, please discuss them at your next appointment.

Safety note — If at any point you develop sudden difficulty controlling your bladder or bowel, numbness between your legs, or rapidly worsening weakness in your legs, seek medical attention the same day. Do not wait for your next appointment.

The Osteospinal story

There have been many years of thought and reflection leading up to the notion and development of Osteospinal. Many hundreds of patients with varied ailments from head to toe and a great deal of back pain. I was originally going to say that much of it was simple mechanical back pain but the reality is no back pain is simple and some is exceptionally complex.

As a clinician my role is to ask a series of questions to establish the cause of the pain, which is not always obvious and varies enormously from person to person. Accurate clinical reasoning is challenging and it's not unusual to find people have been suffering with back pain for years or decades and despite having seen multiple therapists feel that they haven't made progress.

My hope is that Osteospinal can support clinicians in their judgement, especially in difficult to solve puzzles surrounding lower back pain, by giving greater structure to the process.
